Shangri-La is a Health Related Social Needs (HRSN) Housing Support provider in partnership with PacificSource Community Solutions.
What are health-related social needs?
Being healthy requires more than regular check-ups, like having access to non-medical things such as housing, healthy food, and air conditioning, to name a few. Certain Oregon Health Plan (OHP) members may be eligible for extra non-medical services through a program called health-related social needs (HRSN). This name refers to barriers to health such as housing or food insecurity that may affect well-being. ​

What HRSN Services does Shangri-La provide?
Currently Shangri-La can assist with HRSN Housing Supports, specifically rent and utilities for up to 6 months.
Who qualifies for HRSN Housing Supports?
Not all OHP members will qualify for HRSN housing benefits.
HRSN housing benefits aren’t meant for emergencies.

To qualify for rent or utilities assistance, OHP members must:

Be currently housed

Need support staying housed

Have a lease or written agreement with their landlord

Have an income that is 30% or less than the median income in their area

Lack sufficient resources or support networks to prevent homelessness

Have a housing clinical risk factor

Due to a high volume of HRSN requests, processing and response time may be extensive.
How do I access HRSN Housing Support?
Do not contact Shangri-La directly about HRSN Services.

OHP members need to work with their health care provider, CCO, or a local OHP community partner to see if they qualify and apply.

PacificSource members can also complete the HRSN self-referral form online.
​
Once Shangri-La receives your referral, a member of Shangri-La's HRSN team will contact you.
